I have stumbled across some excellent Quad documentation from the BBC. They are as follows:-

(1) Quadraphony: Techniques involved in four channel recording and reproduction.

This article is superb. It explains how the BBC got the Matrix H name, and compares SQ QS with various home grown matrices. Interesting, for me at least, is the fact they used Sansui QSE-4 and QSD-4 encoder/decoders.
