No idea. But since this single was released in 1989, I'd be amazed if it were actually quad.

Killdozer would be awesome in Quad. But I think it's pretty safe to say no.

Nah, that's an indie single release, you see mockup/silly stuff like that all the time. A lot of bands rip off label designs and band logos to their own ends. And since they're hardly popular, can get away with it. Reminds me of Jan & Dean's "Quasi-Modo Mono" on one of their '70s compilations. Flat mono, of course, but somebody thought it was cute.
